Declaration for movement within Bulgaria. Form and example of filling

Starting from March 13, 2020, due to the spread of coronavirus in Bulgaria and the introduction of a state of emergency ( link to document), the government has limited the movement of citizens around the country. Today, at the exit/entry from regions and some cities, checkpoints have been installed where you are required to present a completed declaration form (“declaration for preminavane”). In this article you can download the declaration form in the format pdf and doc, and Also see sample filling.

It should be recalled that in accordance with the order of the Minister of Health, the commandment РД-01-143/20.03.2020 about the introduction of new anti-epidemic measures, the entry and exit of citizens through the checkpoint is allowed only in case of urgent need.

Reasons for passing through the checkpoint

When filling out the declaration, it is worth considering that a serious reason is required to pass through the checkpoint. Unfortunately, “visiting” a store or “walking” is not such a thing. Below we offer you a list of options that most often suit the guards:

  • visiting a personal doctor (visiting a personal doctor)
  • visit a pharmacy (put to the pharmacy)
  • care for the elderly (help for growing up)
  • visiting a dentist (visiting a doctor)
  • change of place of residence (change to place of residence)
  • travel for work purposes, a document from the employer will be required

Checkpoint workers collect declarations for further data processing. Remember that providing knowingly false information when filling out a declaration may entail punishment (Part 355, Al. 2 from the “Punishment Code for Violations”) in the form of a fine 10-50 thousand lv., as well as imprisonment for 5 years.
